The six main organs in Pathogenic, in progression order, with only mechanics confirmed by official achievements and developer updates.

The six regions are listed in progression order, but the useful question is what each region asks the build to prove. Early rooms expose a broken chain; later organs punish weak movement, routing or single-target damage.

Room layouts and rewards can vary between runs. Use the map and room indicators, preserve a working core before entering a boss route, and treat the Brain as separate secret content rather than a seventh main organ.

  • Skin: validate that the main attack, power source and movement plan work together.
  • Intestine and Stomach: protect health and routing choices before spending on optional rewards.
  • Liver and Lung: keep enough range, movement and field awareness for stronger enemy mixes and airflow hazards.
  • Heart: bring a repeatable single-target answer; clearing ordinary rooms is not the same test as clearing the boss.

    Lung

    The May 28 developer preview documented airflow-based hazards and attacks.

  6. Region 6

    Heart

    The sixth main region and the target of multiple run-clear achievements.

Brain is secret content and is not counted among the six main progression regions. Official achievements confirm its name but hide its entrance requirement.
