Start a new run and search the Skin for a secret room. The entrance is hidden behind a wall that reacts when attacked. Because Pathogenic uses generated rooms, copy the visual language of the wall rather than expecting it at the same map coordinate shown in a video.

Step 3: power the tracker before searching the next organ

The tracker is not a passive compass. It needs a working connection to a Mitochondrion and enough Overcharge to activate. Build around that requirement before replacing parts for damage. A strong run that cannot power the tracker cannot finish the route.

Step 4: collect the four yellow shapes

You need one yellow shape from each main organ after Skin: Intestine, Stomach, Liver and Lungs. In each region, charge the tracker, follow it to the hidden route, find the symbol wall and enter the same fixed six-key code. Pick up the yellow object at the end of the room.

Step 5: solve the shape puzzle after the Heart

Defeat the Heart without ending the run. In the sealed room above the exit, you will see a row of grey shapes. Those shapes tell you the order; the four arrow clues from the hidden rooms tell you which direction belongs to each shape.

Step 6: enter the Brain and finish the hidden ending

Move through the opened passage instead of taking the normal ending. The next region is the Brain. It is a full combat area, not a reward room, so the route is only complete after you survive the region and defeat its boss.
